Welcome to Gaydio & The Gaydio Academy's Parent or Guardian Consent Form

Please see below the form and then underneath that the area which you can fill out to confirm you are happy for the young person you are a parent or guardian for taking part in Gaydio or The Gaydio Academy's courses/projects or other volunteering commitments.

PARENTAL CONSENT FORM

FOR APPLICANTS OR VOLUNTEERS 17 & UNDER

I hereby give permission for my child, or the person for whom I am guardian, to take part or become an active volunteer for Gaydio and The Gaydio Academy.

I confirm that they are 17 years of age or under and I, as parent or guardian, confirm they are allowed to take part in any Gaydio or Gaydio Academy courses, projects or any form of volunteering from this point onwards.

Gaydio and The Gaydio Academy aims to provide a safe and enjoyable experience for every child or young person.

To help us do this, please note the following important information:

Parents and guardians must ensure they notify us of any changes to the information given on the form.
Parents and guardians must arrange for children to be brought to and from the activity safely and on time.
We cannot take responsibility for any damaged personal items during the activity.
Parents and guardians understand this information will be kept as a record.
Parents and guardians accept by signing this form that Gaydio and Gaydio Academy staff can administer any medical treatment that my child may need to be given in an emergency

Parents and guardians will let Gaydio or The Gaydio Academy know if they wish their child to not to be filmed or photographed within the activity. This can be done by emailing lee.barnes@gaydio.co.uk

Please note if not emailed then photos or videos of the child or young person may be used on Gaydio and The Gaydio Academy’s website and socials, also for publications or marketing publicity. Gaydio and The Gaydio Academy will take all steps to ensure these images or videos are used solely for the purposes for which they are intended.

Parents and guardians will let Gaydio or The Gaydio Academy know if they wish that audio or video content that their child has created should not be used on air, on the website or social media. This can be done by emailing lee.barnes@gaydio.co.uk
